Aga Khan Health Services (AKHS) is one of the most comprehensive private, not‐for‐profit health care systems in the developing world with community health programs in large geographical areas in Central and South Asia, as well as in East Africa.

The Aga Khan Health Services, Afghanistan (AKHS, A) was established in 2003 as part of the Aga Khan Development Network (AKDN). AKHS, A provides health care services mainly on behalf of the government including the Ministry of Public Health’s of Afghanistan’s Basic Package of Health Services (BPHS) and Essential Package of Hospital Services (EPHS). AKHS, A is unique both in scale and scope, delivering critical health services along the continuum of care, from Community Health Workers (CHWs) to ISO certified hospital facilities.

AKHS, A is currently implementing healthcare services in Bamyan, Baghlan, Badakhshan, Kabul and Takhar provinces. The agency’s scope of work includes the provision of primary and secondary healthcare, preventive and curative medical care, Nursing and Midwifery education, Human Resources Development and Hospital Management. As BPHS and EPHS implementer, AKHS, A provides technical support as well as capacity building of health personnel, immunization services, nutrition trainings and the training of Community Health Nurses and Community Health Midwives.

AKHS, A draws on internal and other donor fundings to continue supplementary and complementary support to the national health system. Moreover, AKHS, A has introduced innovations, including, expansions of e-Health services, e-learning initiatives, and cross border arrangements. To promote healthy environment, AKHS, A is also implementing net zero and greenhouse gas emission initiatives.

تعین نمودن وزن، قد، سن و موک تمام اطفال تحت دوسال و راجستر نمودن انها .

تصنیف نمودن اطفال تحت کتگوری سالم، سوی تغذی حاد متوسط و سوی تغذی حاد شدید.

مشوره دهی به مادران درمورد پرکتس های IYCF و جلوگیری از انتانات.

اگرطفل سالم بود مادر باید به ادامه پرکتس فعلی تشویق گردد.

اگرطفل سوی تغذی حاد متوسط یا شدید داشت باید برایش مشوره دهی IYCF داده شود.

 اجرای نمایش غذایی اگر ممکن باشد.

اگر طفل مصاب به سوی تغذی حاد باشد باید به داکتر رجعت داده شود.

اطمینان از گرفتن ویتامین E در کمپاین گذشته، درصورت که ویتامین E نگرفته باشد باید برایش داه شود.

مشوره دهی در جریان حاملگی و شیردهی.

ارایه مشوره دهی به خانم های حامله در مورد تغذی مناسب در جریان حمل به شمول گرفتن تابلیت آهن و فولیک اسید به شکل منظم.

اطمینان از گرفتن مقدار کافی تابلیت آهن و فولیک اسید خانم های حامله.

ترویج استفاده نمک آیودین دار خصوصا در این مرحله.

به سطح جامعه

کار مشترک با سوپروایزر صحی جامعه و رضاکاران صحی جامعه جهت حمایه تخنیکی در جریان ملاقات ماهانه .

اجرای نظارت رشد ونموی اطفال تحت دوسال و مشوره دهی IYCF به اساس رهنمود تغذی به سطح جامعه.

ارایه مشوره دهی به خانم های حامله در مورد تغذی مناسب در جریان حمل به شمول گرفتن تابلیت آهن و فولیک اسید به شکل منظم.

انتقال حد اقل 1-2 پیام تغذی به جامعه در هرماه .

حمایه ورهنمای به سوپروایزصحی جامه/ رضاکار صحی جامعه جهت ملاقات فامیلی اطفال که به شکل درست رشد ونمو نمیکند و یا هم مصاب به سوتغذی باشد و ضرورت به حل مشکل احساس میگردد.

ریکارد و راپور

حفظ منظم راپورها تا خدمات عرضه شده به آسانی ردیابی و مانیتور گردیده به شمول بخش تغذی کتاب طفل و مادر.

مشخص کردن مستفدین و شریک ساختن معلومات آن با سوپروایزر صحی جامعه که ضرورت به تعقیب داشته باشد.

حاضر شدن به وظیفه مطابق رسمیات کلینک.

تهیه پلان کاری ماهوار در شروع ماه.

اشتراک درجلسه صبحانه کلینک و ارایه گذارش.

وزن، قد و موک نمودن اطفال تحت دوسال.

به اساس انتروپومتری، تفسیر وضعیت تغذی طفل و رسم گراف آن در کارت. اگر طفل بار اول مراجعه نموده باشد باید برایش کارت نو داده شود.

بر اساس وضعیت تغذی طفل که در کارت نشان داده شده به مادران اطفال مشوره های مربوطه ولازمه تغذی جهت بهبود تغذی اطفال داده شود

ارایه مشوره های لازمه تغذی در حاملگی و شیرده به مادران و اطمنان از گرفتن مقدار کافی تابلیت آهن و فولیک اسید ( 30 تابلیت درماه).

رجعت اطفال که در سکریننگ سوی تغذی دریافت میگردد به داکتر کلینک ( اطمنان از اینکه طفل به داکتر مراجعه نمود)

فهماندن مادر در مورد ملاقات بعدی و تاکید به مراجعه بعدی.

راجستر نمودن اطفال در راجستر نظارت رشد و نمو.

هماهنگی با سوپروایزر صحی جامعه در مورد پلان ساحوی و تعقیب اطفال که رشد ونمو نمیکند.

اشتراک در جلسه رضاکاران صحی جامعه به اساس پلان.

تهیه راپور ماهوار فعالیت خویش در آخر ماه.

رجعت خانم های حامله وشیرده که امراض پیشرفته ثدیه ویا هم سایر مشکلات نسایی وولادی داشته باشد.

رجعت اطفال به بخش واکسین.

ارایه تعلیمات صحی به اساس پلان ماهوار کلینک.

اشتراک در جلسه ماهوار شورای صحی کلینک.

تحصیلات:

فارغ از صنف 12 با داشتن تریننگ تغذی ُو یا تعلیمات متوسط.

تجربه کاری:

داشتن یک سال و یا شش ماه تجربه کاری در بخش صحت.

در صورت داشتن تجربه کاری مرتبط، اولویت داده خواهد..

مهارت:

آشنایی کامل به زبان های محلی (دری و پشتو)

آشنایی با اصول سکریننگ تغذیه، طبقه‌ بندی سوء‌تغذیه و مشاوره تغذی.

مهارت هماهنگی و کار گروپی
