Job Description:
The nurse at Hazarnaw BHC is responsible for providing nursing care, nutritional support, and pharmacy assistance to patients, including maternal, newborn, and child health services. The Nurse works under the supervision of the Medical Doctor and in collaboration with midwives, MHPSS counselors, community health supervisor (CHS), CHWs, and other staff to ensure quality, accountability, and adherence to BPHS standards, nutrition protocols, and child protection principles.
Main responsibilities:

Provide nursing care to patients at the BHC: assess health status (vital signs, symptoms, history), assist in primary care consultations, administer medications and treatments under physician/clinical protocol.

Monitor patients’ progress, maintain accurate nursing records (assessment forms, vital signs charts, input/output logs), and communicate changes to the supervising clinician.

Ensure infection prevention & control practices are followed (sterilisation, hygiene, waste management) in the facility.

Participate in health education of patients & families: explain illness, treatments, follow-up, self-care, nutrition implications.

Support outreach or community health activities as required: working with community health workers (CHWs) and community health posts to promote uptake of services.

Attend meetings (e.g., mortality & morbidity review, infection control committee) and work to improve the quality of care and patient safety.

Do screening of children under five.

Classify children in normal, Moderate Acute Malnutrition (MAM), and Severe Acute Malnutrition (SAM) categories.
-
The SAM cases should be referred to the doctor for adequate treatment.
-
Administer Vitamin A if the child has not received Vitamin A supplementation during the previous NID.

Promote consumption of iodized salt.
-
Triage the patients and provide first aid.
-
Manage shock, including open vein and control of bleeding.
-
Wound care and dressing.
-
Identification of fracture, including neck, spinal cord, and its stabilization/immobilization.

Cooperate with the doctor in the distribution of medicine to the patients.

Assess the psychosocial status of affected people.

Provide psychosocial counselling for individuals and groups.
-
Refer complicated cases to a doctor or higher levels, and follow up on cases.

Provide weekly and monthly reports.
Nutrition-Specific Duties
-
Conduct nutritional screening and assessment of target groups (e.g., children under five, pregnant & lactating women (PLWs), caregivers) using accepted criteria (e.g., MUAC, weight-for-age, clinical signs).

Identify children with Moderate Acute Malnutrition (MAM) and Severe Acute Malnutrition (SAM), pregnant/lactating women at risk of under-nutrition, in alignment with the nutrition component of BPHS or the implementing partner’s protocols. Develop, implement, and monitor individualized nutrition care plans or feeding support (e.g., counselling, supplementation, therapeutic or supplementary feeding) in collaboration with a dietician/nutrition officer and the nursing team.

Administer or oversee enteral/parenteral nutrition (where applicable) or feeding formulas, supervise feeding assistants, and monitor for complications (aspiration, intolerance).

Provide nutrition counselling and education to mothers, caregivers, families, and community groups on healthy diet, infant & young child feeding (IYCF), maternal nutrition, hygiene & sanitation, linkages with other services.

Ensure availability, proper storage, management, and documentation of nutrition supplies (e.g., RUTF, RUSF, Super Cereal) and medications, and that stocks are maintained and pre-positioned as required. (As noted in a sample job description: “Ensuring adequate availability of nutrition supplies and medicines and ensuring stock is pre-positioned as planned.”)

Monitor outcomes: weight gain, nutritional status changes, treatment outcomes, referrals of complicated cases (e.g., SAM with complications) to Therapeutic Feeding Units (TFUs) or higher-level facilities. A sample job listing: “Referring complicated SAM and MAM cases to TFUs.”

Prepare and submit nutrition reports and documentation (weekly/monthly) to the facility in charge and nutrition supervisor, flag any issues or stockouts, and participate in review meetings.
Child Protection Responsibilities

Identify and referred child protection cases to the social worker in accordance with child protection protocols.

Ensure child-friendly practices in BHC activities, including privacy, dignity, and safety during consultations and treatments.

Promote awareness of child rights, protection principles, and safe health practices among staff, patients, and caregivers.
