Médecins du Monde - France (MdM-F) is an international humanitarian organization whose mission is to provide medical care for the most vulnerable populations, the world over, including Afghanistan. It seeks to stimulate voluntary commitment from doctors, other health care providers, and from those whose expertise in other fields is needed for its activities, to enlist all competent support required for the achievement of its projects, and to seek at all times to encourage close working relationships with populations in its care.

KEY DUTIES AND RESPONSIBILITIES:

Main Duties & Responsibilities

• Conduct nutrition screening of all children 0-59 months.
• Conduct growth monitoring of all children under two years and register them.
• Conducting the GM of under 2y and on-the-spot counselling to mothers of under 2y children’s.
• Responsible for overall management of acute malnutrition.
• Food demonstration at Health facility
• Prepared the monthly IYCF and GM report
• Conduct appetite tests for all beneficiaries identified as SAM and MAM before registration into the nutrition program and during follow-up.
• Registration of beneficiaries into OPD SAM and MAM program and frequent follow-up until exit from the program as per the national IMAM guidelines.
• Fill out the admission card for SAM and MAM children.
• Regularly conduct nutrition education sessions for caregivers of malnourished children  
• Regular collection of nutrition data at the health facility level.
• Development of a nutrition filling system at the health facility level for all data collection tools.
• Regular distribution of nutrition supplies (RUTF, RUSF, and Super cereal plus/LNS) for SAM and MAM treatment.
• Timely submission of therapeutic food distribution reports on a weekly, Bi-weekly, and monthly basis.
• close follow-up of nutrition commodities consumption with pharmacy technicians or drug dispensers, and provide reports to the office team in case of shortage, near expiration, etc.
• Timely preparation and submission of monthly statistics, biweekly, and stock reports.
• Participation and leading the nutrition team during the mass MUAC screening campaign every quarter.
• To accurately and appropriately treat or refer beneficiaries in the nutrition program by adhering to national and international guidelines and organizational procedures.
• Discharge children according to specified discharge criteria and national protocol and guidelines.
• Ensure accurate record keeping of all nutrition activities, filling in forms, cards, registration books, and tally sheets as required.
• Compile reports as required and requested by the Health and Nutrition supervisor.
• Referred all SAM children with complications according to IMAM guidelines to the nearest TFU with proper documents of registration, refer sheets, and cash voucher in case cash service was available, with close coordination of HFs/MHNTs in charge, and other documentation is needed.
• Regularly fill out the register book during patient visits, documenting all services provided and advice given, in line with nursing responsibilities
• Assist the doctor in categorizing and examining patients
• Collaborate with different departments such as the vaccination unit, maternal and child health, etc., to improve service delivery
• Actively participate in health education sessions for clients
• Prepare monthly reports on all dressing room activities
• Ensure proper storage and appropriate use of all equipment and materials
• Coordinate with health shura members and community health workers (CHWs) to strengthen community health activities and organize monthly meetings
• Adhere to infection prevention protocols and sterilization procedures using personal protective equipment
• Prepare chlorine solutions with appropriate concentrations for all departments
• Responsible for sterilizing medical instruments in the health facility
• Identify suspected tuberculosis cases and refer them to the TB unit
• Collaborate with the nutrition counselor in screening and growth monitoring
• During medication distribution, provide clear instructions to patients on how to use their medications, ensure proper storage, and record them in the appropriate forms
• Control and monitor pharmaceutical items in terms of expiry date and physical appearance. Medicines that have six months remaining before expiry must be reported to the Clinic In‑Charge (Head of Clinic).
• Medicines must be stored and maintained in the pharmacy depot according to standard guidelines.
• Medicines should be consumed according to (FIFO), (FEFO).
• Proper registration of the depot stock card is the responsibility of the pharmacist, including: medicine name and description, date, dosage, and monthly movement of medicines in and out of the pharmacy depot.
• The daily consumption of medicines must be accurately measured and recorded.
• Accurately measure and record daily medication consumption
• Have sufficient knowledge of the BPHS and apply it in practice
• Perform any other duties assigned by the relevant supervisor or Médecins du Monde France (MdM-F) management

Accountability:

• Provide awareness raising on feedback and complaints mechanisms and channels available to every patient/beneficiary;
• Timely inform line managers on channel unavailability or damage at HFs level (IEC material, speakers, etc.)
• Adhere to MdM F organizational policies.

مسئولیت مدیریت کامل سوء‌تغذیه حاد

نمایش تهیه غذا در مرکز صحی

اجرای تست اشتها برای تمام کودکان مبتلا به سوء‌تغذیه شدید و متوسط قبل از ثبت در برنامه تغذیه و در جریان تعقیب

ثبت کودکان در برنامه  و و تعقیب منظم تا زمان رخصت طبق رهنمودهای ملی

برگزاری منظم جلسات آموزشی تغذیوی برای مراقبین اطفال مبتلا به سوء‌تغذیه

جمع‌آوری منظم ارقام تغذی در سطح مرکز صحی

ایجاد سیستم منظم فایلنگ تغذی در مرکز صحی برای تمام وسایل جمع‌آوری ارقام

ارسال به‌موقع گزارش‌های توزیع غذاهای برای تداوی ‌صورت هفته ‌وار، دو‌هفته ‌وار و ماهانه

تعقیب دقیق مصرف مواد تغذیوی با مسول دواخانه یا توزیع ‌کننده دوا، و ارائه گزارش به تیم دفتر در صورت کمبود، نزدیک بودن تاریخ انقضا و غیره

تهیه و ارسال به ‌موقع احصائیه ‌های ماهانه، گزارش‌های دو‌هفته ‌وار و گزارش‌های ذخایر

اشتراک و رهبری تیم تغذیه در کمپاین سکریننگ هر سه ماه

تداوی یا ارجاع دقیق و مناسب کودکان در برنامه تغذیه‌ای طبق رهنمودهای ملی و بین‌المللی و طرزالعمل‌های سازمانی

رخصت اطفال طبق معیارهای مشخص ‌شده و پروتوکول‌ها و رهنمودهای ملی

اطمینان از ثبت دقیق تمام فعالیت‌های تغذیوی، تکمیل فورم‌ها، کارت‌ها، کتاب‌های راجستر و فورم‌های جمع‌بندی طبق نیاز

به طور منظم در طول ویزیت‌های بیمار، دفتر ثبت را پر کنید و تمام خدمات ارائه شده و توصیه‌های داده شده را مطابق با مسئولیت‌های پرستاری ثبت کنید

 کمک به پزشک در دسته‌بندی و معاینه بیماران

 همکاری با بخش‌های مختلف مانند واحد واکسیناسیون، بهداشت مادر و کودک و غیره، برای بهبود ارائه خدمات

مشارکت فعال در جلسات آموزش سلامت برای مراجعین

تهیه گزارش ماهانه از تمام فعالیت‌های رختکن

اطمینان از نگهداری صحیح و استفاده مناسب از تمام تجهیزات و مواد

هماهنگی با اعضای شورای سلامت و کارکنان صحی جامعه برای تقویت فعالیت‌های بهداشتی جامعه و برگزاری جلسات ماهانه

رعایت پروتکل‌های پیشگیری از عفونت و رویه‌های استریل سازی با استفاده از تجهیزات حفاظت فردی

تهیه محلول‌های کلر با غلظت‌های مناسب برای همه بخش‌ها

مسئول استریل کردن ابزار پزشکی در مرکز درمانی

موارد مشکوک به سل را شناسایی و آنها را به واحد ارجاع دهید

همکاری با مشاور تغذیه در شناسایی و پایش رشد

در طول توزیع دارو، دستورالعمل‌های واضحی در مورد نحوه استفاده از داروهایشان به بیماران ارائه دهید، از نگهداری مناسب آنها اطمینان حاصل کنید و آنها را در فرم‌های مناسب ثبت کنید

اقلام دوایی را از نظرتاریخ وشکل ظاهری آنها کنترول و تعقیب نمایید ، آنعده ادویه جاتی که 6 ماه به ختم تاریخ شان مانده به آمر کلینیک باید خبر دهد

ادویه را در دیپوی فارمسی مطابق رهنمای ستندرد حفظ و نگهداری نماید

ادویه که اول وارد شده، اول خارج شود. ادویه که اول منقضی‌شده، اول خارج می‌شود

راجستر نمودن کارت دیپو به صورت مناسب: مانند نام و توضیحات دوا، تاریخ، دوز دواو انتقال ماهانه دوای داخل دیپوی ادویه، مسولیت فارمسیست میباشد

مقدارادویه جات که روزمره بمصرف میرسد، باید بصورت دقیق سنجیده و ثبت نماید

از جابجانمودن وطرزنگهداشت سامان و وسایل داخل فارمسی وادویه جات بصورت پاک، محفوظ و مناسب اطمینان داشته باشد

اندازه‌گیری و ثبت دقیق مصرف روزانه ادویه

دانش کافی در مورد داشته باشید و آن را در عمل به کار ببرید

انجام هرگونه وظیفه دیگری که توسط مسول مربوطه یا مدیریت پزشکان جهان فرانسه تعیین می‌شود

حمایت از تنظیم انتقال در صورت نیاز (ترانسپورت، اسناد و غیره)

مسئولیت‌پذیری

ارائه آگاهی‌دهی در مورد مکانیزم‌ها و کانال‌های دریافت شکایات و جواب دهی برای هر مریض/مستفید

اطلاع‌دهی به موقع به مسولین مربوطه در صورت عدم دسترسی یا آسیب‌دیدگی کانال‌های ارتباطی در سطح مراکز صحی (مواد، بلندگوها و غیره)

رعایت کامل پالیسی‌های سازمانی

Mandatory qualifications/requirements: 

• Bachelor of Nursing with at least one year working experience and Passed Nursing Exit exam.
• Diploma/Certificate in Nursing or Nutrition public health.
• Language: Fluency in local languages.
• Local residents of Nimruz province will be preferred.

Relevant experience

• At least 1 year of experience in nutrition, and or at least 2 years of experience in the health sector.
• IYCF counselling experience
• MIYCN training
• IMAM training
• Psychosocial support counseling skills or experience
• Local residents of Nimruz province will be preferred.

تسلط کامل به زبان‌های دری و پشتو – مهارت گفتاری و نوشتاری الزامی اس

باشندگان محلی ولایت نیمروز در اولویت قرار دارند
