Job Description:
Responsible for preparing, scanning, indexing, and uploading patient records, as well as retrieving files from storage and departmental locations to ensure timely access and continuity of patient care.
Job Requirements:
- Regular Responsibilities
- Oversee the preparation process of key medical record documents.
- Review scanned documents to ensure accuracy and completeness.
- Ensure the overall quality and integrity of medical records.
- Identify errors or discrepancies in medical records, resolve issues, and ensure timely uploading to the HIMS portal.
- Detect and correct errors in both scanned and prepped medical records.
- Maintain high-quality scanned images while meeting the productivity target of 400 medical records per day with high accuracy.
- Establish and monitor productivity (400 files/day) and accuracy standards for document preparation.
- Ensure medical records are readily accessible to healthcare providers to support timely patient care.
- Retrieve files from storage based on inactive file lists to support the document imaging process.
- Prepare files according to the key document checklist.
- Perform daily scanning of prepared files.
- Index scanned files in the system by creating batches using Visit IDs for both inpatient and outpatient encounters to enable efficient retrieval.
- Upload scanned files to the server to ensure availability and continuity of patient care.
- Retrieve files from inactive storage areas as required to support ongoing operations.
- Perform any other related duties assigned by the supervisor.
- Periodic Responsibilities
- Transfer inactive files to the scanning area as needed to optimize space in active filing areas.
- Ensure medical records are scanned efficiently to enhance accessibility at the point of care.
